Our Team

Raphael and Britt Akobundu

Blue Monarch Hospice was founded by husband and wife nurses looking to improve both end-of-life care and the treatment of healthcare workers. Raphael and Britt met while working on the same floor at Scripps Encinitas Memorial Hospital. Raphael decided to go back to school for his master’s in nursing due to burnout and in hopes of venturing out on his own. Britt pursued her calling in the operating room which is where she currently works as an OR nurse. During Raphael’s time at the hospital, he noticed a gap in customer service and patient care in regards to patients transitioning to hospice care, which is why Raphael and Britt decided to open Blue Monarch. Raphael and Britt are also huge advocates of work-life balance amongst healthcare workers and they believe healthcare workers provide the best care when they feel supported and not overworked.

Hospice Medical Director

Our medical director coordinates our team of nurses, physicians, chaplains, health aides, and volunteers to ensure the needs of your loved one and family are met.

Registered Nurse Care Manager

Your Registered Nurse (RN) Case Manager oversees your loved ones’ medication and symptom management. This includes making improvements to your loved one’s quality of life.

Certified Home Health Aide

Your Certified Home Health Aide works closely with your RN and is responsible for assisting with meals, walking, moving your loved one from a bed to a chair and vice versa, and in helping them become comfortable in their bed. They also assist with activities of daily living such as bathing, toileting, eating, dressing and grooming.

Medical Social Worker

The role of your Medical Social Worker is to create an overall care plan for your loved one. This includes educating you and your family members on the type of care needed to help your loved one become comfortable, providing you and your family members with specific ways you can help, listening to your concerns and fears, and keeping you updated on the physical and emotional state of your loved one.

Chaplain

Your Chaplain assists with the end-of-life process through a spiritual and emotionally comforting approach.

Volunteers

Volunteers will assist you and your family with daily chores, playing games, talking, and overall providing social interaction.

Primary Caregiver

The Primary Caregiver is typically selected by you and your family and is available to provide day-to-day medical support and guidance for your loved one.
