Continuous Home Care is an essential level of hospice care that provides round-the-clock nursing, medical and psychosocial care to patients who require more intensive symptom management than Routine Home Care can offer. Blue Monarch Hospice understands the importance of providing Continuous Home Care to ensure that patients receive the necessary medical attention to manage their symptoms effectively. This type of care may be required for a patient experiencing severe pain or other symptoms that cannot be adequately managed through Routine Home Care. In such cases, the patient may require continuous monitoring and frequent interventions to manage their symptoms and ensure their comfort.

Blue Monarch Hospice’s team of experienced healthcare professionals provides personalized and compassionate care to meet each patient’s unique needs. The team collaborates to deliver advanced symptom management, emotional, and spiritual support, and the necessary medical attention to ensure the best possible quality of life for patients. The hospice care team also works closely with the patient’s primary caregiver to ensure that they are fully informed and equipped to manage the patient’s needs when the hospice team is not present.
Continuous Home Care is one of the four levels of hospice care offered by Blue Monarch Hospice, along with Routine Home Care, Continuous Home Care, and General Inpatient Hospice Care services. With a focus on holistic and patient-centered care, the hospice team provides the highest level of care and support to patients and their families throughout the end-of-life journey. The team understands that hospice care can be challenging for patients and their families and works tirelessly to provide comfort, care, and emotional support throughout this difficult time.
